Improve Health Equity for Your Community
Acclivity Health Can Help You Succeed with ACO REACH
ACO REACH On-Demand Webinar
More data. Less paperwork. Better care.
ACO REACH will replace the Global and Professional Direct Contracting (GPDC) payment model beginning on January 1, 2023. It is designed to be a more provider-led program for:
Standard ACOs with experience serving Original Medicare patients.
New Entrant ACOs who may rely initially on voluntary alignment and claims-based alignment.
High Needs Population ACOs that serve high-risk Original Medicare patients with complex needs who are aligned through voluntary or claims-based alignment.
If your practice or ACO is currently a GPDC Model participant, you don’t need to submit a new application. However, you must maintain a strong compliance record in 2022 and accept the requirements for the ACO REACH model by January 1, 2023.
