Title: Artificial Intelligence in End-of-Life Care – A Bridge Too Far?
Presenter: Jeremey Powell, CEO, Acclivity Health
Abstract: In an increasingly tech-savvy world, healthcare is often one of the last sectors to fully adopt innovative techniques. This education session seeks to improve the understanding of what Artificial Intelligence is, and why it is crucial for end-of-life settings. Powell will outline the ways Artificial Intelligence can be used to improve patient outcomes, lower practice costs and increase practitioner efficiencies.
Title: Utilizing Level of Need as a Determinant for Level of Care
Presenters: John Mulder, MD, Medical Director, Trillium Institute and
Robin Stawasz, Program Development Executive, Acclivity Health
Abstract: Since the inception of hospice, prognosis has been used to determine eligibility. But experience has shown that prognosis is not always a good indicator of the level of need. Now, data analytics and predictive modeling allow for objective and consistent determination of patient need. Robin will discuss how tying the level of service to the acuity of need permits a continuity of care and a responsive reimbursement system that could transform post-acute care.
