Collaboration enables primary care practices to participate in ACO REACH model and provides data and insights to help improve care quality and increase revenue

The Centers for Medicare & Medicaid Services (CMS) recently announced it will launch the newly developed Accountable Care Organization Realizing Equity, Access, and Community Health (ACO REACH) model in January 2023. A redesign of the Global and Professional Direct Contracting model, ACO REACH aims to increase participation from primary care physicians (PCPs) and other eligible professionals in value-based payment models. This shift to value-based care can be daunting for primary care practices, many of which are under-resourced and under-staffed.
