Hospices Gauge How Quality Counts in ACO REACH Payment

Recently, Acclivity hosted a webinar on the newest value-based care payment model from the Center for Medicare and Medicaid Innovation Center: ACO Realizing Equity, Access, and Community Health Model—or ACO REACH.  This program is a replacement for the Global and Professional Direct Contracting (GPDC) payment model. ACO REACH is a more provider-led ACO model, designed to promote health equity and minimize healthcare disparities for underserved communities.  Holly Vossel of Hospice News was in attendance at our session and provided commentary from our COO, John Dickey in her article, Hospices Gauge How Quality Counts in ACO REACH Payment.
